Therapeutic Lifestyle Changes
Program Details (Back to Programs)

The Therapeutic Lifestyle Changes (TLC) program is offered to patients with high cholesterol (dyslipidemia) and/or high blood pressure (hypertension).

During these visits patients will meet with a Registered Nurse and a Registered Dietitian to discuss lifestyle changes necessary to reduce high cholesterol and blood pressure.  Patients will develop individualized goals and targets in collaboration with the healthcare team. Some of the lifestyle changes may include:

  • Healthier eating habits
  • Increase in physical activity
  • Stress reduction
  • Smoking cessation
  • Healthy sleep patterns 

The purpose of the TLC program is to provide individualized education and support for patients willing and motivated to participate in a supportive health education program.  Patients can expect the initial visit to last approximately one hour. Follow up visits will occur every three months depending on personal needs.
