Two Rivers Family Health Team provides primary health care, including chronic disease management, health promotion and disease prevention, to a rostered population of patients in the local community and surrounding area. Two Rivers Family Health Team employs an interprofessional team of health care providers who work in a collaborative model of care.

Two Rivers Family Health Team is currently seeking a 0.8 FTE Chiropodist OR Advanced Foot Care Nurse (4 days/week) to join its growing team of health care professionals. We are an equal opportunity employer and welcome interest from all qualified applicants.

The Chiropodist/Advanced Foot Care Nurse will be responsible for triaging and implementing foot care services for FHT patients referred from primary care and existing patients within other TRFHT programs who meet referral guidelines. The range of services will include active treatment of acute pathological foot conditions, preventive and palliative foot care and the promotion of healthy feet.

You will work independently and in collaboration with our multidisciplinary team to provide excellent patient health care to a wide variety of patients with a broad spectrum of presenting needs.

Roles and Responsibilities:

  • Provides individual, evidence-based treatment and foot care according to best practices and within the Chiropodist scope of practice as outlined in the Ontario Regulated Health Professionals Act (1991) and the Chiropody Act (1991)
  • Works with the interdisciplinary team to provide collaborative and well-rounded patient care
  • Provides foot care education to meet patient learning needs
  • Facilitates group educational sessions as required
  • Integrates with local community resources and services to enhance coordination and access to care
  • Participates in program planning, quality improvement and quarterly evaluation
  • In collaboration with the Family Physician and other members of the interdisciplinary team, develop and discuss appropriate individualized care plan with the patient/caregiver based on best practice
  • Provide ongoing case management for patients
  • Document all health notes into the EMR and arrange necessary follow-up if required
  • Prepare quarterly statistical data to ensure statistics are reported to the Ministry of Health

Skills/Knowledge Requirements:

  • Excellent clinical skills and experience with assessment, treatment and management of problems commonly seen in primary care
  • Demonstrated ability to engage in collaborative practice with interdisciplinary colleagues both across the FHT and the physician teams
  • Excellent leadership, organization, evaluation, time management, communication and interpersonal skills
  • Demonstrated problem solving skills, good judgement and flexibility.
  • Knowledge of electronic medical records (PSS) an asset, as well as proficiency in the use of MS Office
  • Other duties as required

Education/Job Requirements:

  • Registered and in good standing with the College of Chiropodists of Ontario OR College of Nurses of Ontario
  • Post Secondary education in Chiropody OR Certification from an accredited Advanced Foot Care course
  • Two years of foot care experience working in primary care or community-based setting
  • Demonstrated ability to work collaboratively with physicians, interdisciplinary healthcare providers and other community partners
  • A strong understanding of current chiropody best practices and guidelines
  • Experience with reprocessing, including tool cleaning and sterilization
  • Able to work flexible hours including evenings if required
  • Police Record/Vulnerable Sector Check
  • Current Ontario Drivers’ License
