To find a health care provider in your area please visit Health Care Connect.  Or if you are in the Cambridge, Ontario area, check the Doctors 4 Cambridge website for Doctors and Nurse Practitioners who are accepting patients.

The Health Care Connect online registration tool provides:

  • Round-the clock access to on-line registration – saving time for busy Ontarians
  • Options to register more than one patient – you can register your family and dependants
  • Registration that will connect patients to a Care Connector – a nurse who will help patients without a family health care provider to find one
  • Security features to protect the privacy of your personal health information
  • A link to the Health Care Options website, a medical services directory with a built-in mapping system, for patients to find their local health care options

Your Care Connector and you

After you’ve registered with Health Care Connect, a nurse – called a Care Connector – will be assigned to help you find a health care provider in your area. Your Care Connector will be your main point of contact with the program. Your will receive contact information for your Care Connector by mail after you have registered for the program.

Your Role and Responsibilities

Health Care Connect assists you with your search for a family health care provider. You should have a valid OHIP card to be part of the program.

Health Care Connect is here to help, but it doesn’t guarantee that a family health care provider will be found. You are urged to keep searching on your own for a provider. Be sure to contact your Care Connector if you move, find a doctor or if there are changes in your health needs.

You will be referred to a family physician or a nurse practitioner in your community once one is found. You are then responsible for calling to book an appointment.
