Our Diabetes Education Program (DEP) is for patients and their families with prediabetes and diabetes.  This program will help you live healthy with diabetes.  We offer our DEP services to members of the community in addition to patients of Two Rivers Family Health Team.

Our program talks about healthy food choices, physical activity, education, blood sugar testing, lab testing, and overall quality of life.

Your doctor or other member of your health care team may recommend you attend this program if your blood sugar is higher than normal.  You may also attend this program if you would like more information about your diabetes or prediabetes, or if you would like help managing your diabetes.
