We look forward to helping you reduce your pain, move more easily, and do your daily and leisure activities. For us to provide you with the best possible care, please keep the following in mind:

This is an active physiotherapy program, whereby you will be required to participate in an exercise program at home. This is critical for improving your function and mobility and reducing your pain. Doing your home exercise program is required if you want to continue to see the physiotherapist.

Eligibility Criteria:

  • Referral by a Two Rivers Family Health Team physician or one of our health care providers
  • Patients 20-64 years of age will be seen for 4-8 physiotherapy visits
  • Patients 0-19 or >65 years of age or receiving ODSP/OW will have a maximum of 4 physiotherapy visits

*Patients with extended health insurance, WSIB or motor vehicle accident claims MUST access physiotherapy at a local private practice.

Offers exercise videos and one on one appointments for patients diagnosed with arthritis. Patients can use referral form on the website.

The Lung Health Foundation has developed a model for exercise maintenance for people with chronic obstructive pulmonary disease (COPD), as well as other chronic lung diseases such as severe asthma and pulmonary fibrosis.   Home exercise modules are currently available on the website.
