anticoagulation-management

The INR clinic is offered to patients who require warfarin (or Coumadin) therapy and are having trouble keeping their INR stable.  The INR measures how well the warfarin medication is reacting to the blood to keep it “thin” enough to prevent or treat blood clots, but not to cause bleeds.

Offered at various times throughout the week, each visit is approximately 10 minutes in length. The pharmacy team will manage the INR testing and warfarin dosages at the appointment and also provide detailed information on managing warfarin therapy at home. This includes information about diet and drug interactions, and the signs or symptoms of bleeds or clots.

Please see your primary health provider at Two Rivers Family Health Team for more information and to see if this program is right for you.
