Clinical Pharmacy Program

The role of the Clinical Pharmacy Team at the Two Rivers FHT is to provide patient-centred care in collaboration with healthcare providers and to aid patients in meeting their health-related goals through education and chronic-disease management.

Patients can be referred to the clinical pharmacist for a medication assessment by their primary care provider or through self referrals.  Medication Assessments are a thorough patient interview, chart review and assessment, aimed at optimizing the medication therapy.  Optimal drug therapy considers the use of evidence-based treatment options, prescriber and patient preference, minimization of side-effects and intolerable drug interactions, cost-effectiveness, convenience and manageability of the medication regiment geared towards the patient.  The pharmacist will assess the effectiveness, safety, and compliance of all drug therapies.
