Dear Patients,

I have found it a joy to provide medical care to you over the years. Thank you for allowing me to be a part of so many important life events for you and your families. I am writing to you to let you know that I will be leaving my family practice as I am moving across Canada.

I feel very fortunate in finding Dr. Nicole Carpe to take over my practice effective November 1, 2023 so that you (and all of my patients) will have an option for continued care at HealthLink Family Practice and will not need to search for a replacement primary care provider. I have had the pleasure of working alongside Dr. Carpe as she has been a locum at HealthLink since January 2022. I am pleased that such a caring and competent physician has agreed to join HealthLink Family Practice and the Two Rivers Family Health Organization on a permanent basis.

I recognize that transitioning to another physician may be stressful for some patients, but I strongly encourage you to continue to seek out care as needed. Between now and November, I will continue to be available to you, to provide you with a seamless transition of care.

Your medical records will automatically remain at HealthLink Family Practice with Dr. Carpe unless you provide written authorization to transfer your records to another physician or primary care provider of your choosing. Should you decide to remain at HealthLink and transition to Dr. Carpe, an updated roster form will be provided to you for you and your family to complete following Dr. Carpe’s arrival which will confirm your spot in her practice.

I will miss my patients and colleagues at HealthLink Family Practice and the Two Rivers Family Health Team dearly.

Dear Patients,

I am excited to introduce myself as the newest physician joining HealthLink Family Practice. My name is Dr. Nicole Carpe and I will be taking over care from Dr. Kate Morgan in November 2023.

To provide some background, I grew up in Ontario, obtaining my Bachelor of Science degree at the University of Guelph before living in Montreal where I completed a masters program in Human Genetics at McGill University. I then transitioned to medicine at the University of Toronto and finished with Family Medicine residency training through McMaster University. I settled in the Flamborough area of Hamilton and have been working as a locum family physician for several years now.

When not working I am spending time with my husband and three children. We enjoy the outdoors, skating, swimming and hiking at local conservation areas.

I am looking forward to getting to know each of you and support you along any of your health journeys.
