HealthLink Family Practice – Preform

Only use the below link if you have been asked by a Physician or Staff Member of HealthLink Family Practice to submit a picture or document for the physician to review.

Before proceeding, you will need the Name, Date of Birth and Health Card Number of the Patient that the picture or document is from.

If sending pictures, please do not send more than 4 photos.

You may include PDF documents, image files (JPG, PNG), plain text files (.txt), or standard audio-visual files, up to 10 MB in size.

Graphic / sensitive photos (eg. of region covered by underwear) should not be sent.

Please do not make appointment requests through this website. You must call the clinic and speak with reception.

Note, your picture/document will be reviewed at your next appointment. If your issue needs immediate attention, please call the office to determine if you need immediate care.
