Two Rivers Family Health Team provides primary health care, including chronic disease management, health promotion and disease prevention, to a rostered population of patients in the local community and surrounding area. Two Rivers Family Health Team employs an interprofessional team of health care providers who work in a collaborative model of care.

Two Rivers Family Health Team is currently seeking a 0.5 FTE Psychologist (2.5 days/week) to join its growing team of health care professionals. We are an equal opportunity employer and welcome interest from all qualified applicants.

The Psychologist will be responsible for screening and assessments of patients with behavioural difficulties, social emotional issues, developmental or cognitive delays, and mental health disorders.

You will work independently and in collaboration with our multidisciplinary team to provide excellent patient health care to a wide variety of patients with a broad spectrum of presenting needs.

Roles and Responsibilities:

  • Screening and assessments of patients
  • Processes referrals to determine suitability for assessment.
  • Strong working knowledge of community resources to redirect referrals to appropriate services.
  • Select, administer, score and interpret psychological tests.
  • Review clinical information provided on medical files and health records
  • Conduct comprehensive consultations with patients to obtain relevant information and assess needs.
  • Facilitate access to services and facilitate linkages with appropriate local services, and resources.
  • Collaboration with Physicians, Nurse Practitioners, and other Interdisciplinary Providers regarding patients diagnosis, and resources and referral needs.
  • Collaborate with Family Health Team Social Workers regarding patient progress, therapy resources and community resources to help facilitate change.
  • Take part in Mental Health Team Meetings to help with development of programs to meet patient needs.
  • Collect data as required for statistical and reporting purposes.
  • Liaise with community services and provide appropriate documentation to facilitate referrals and supports
  • Maintain confidentiality of clinical files and document appropriate information on FHT medical files.
  • Completion of applications for supports (ODSP forms, Disability Tax Credits and assist with CPP forms as requested).
  • Promote FHT services and programs to patients.

Skills/Knowledge Requirements

  • Registration with the College of Psychologists of Ontario as a Psychologist or as a Psychological Associate
  • Completed supervised training or to be under the supervision of a licensed psychologist
  • Evidence of professional liability insurance
  • Proficiencies in assessment of children 2-12, adolescents, and adults
  • Knowledge of Autism Spectrum Disorder, Developmental Delays as well as Mental Health issues related to children and adults
  • Ability to conduct screening assessments related to learning and make recommendations to receive further services.
  • Demonstrated knowledge of the Mental Health Act
  • Maintain current knowledge of relevant research and current criteria for diagnosis
  • Leadership and decision-making skills
  • Superior interpersonal, communication and organization skills
  • Knowledge of electronic medical records (PSS) an asset, as well as proficiency in the use of MS Office
  • Police Record/Vulnerable Sector Check
