Two Rivers Family Health Team provides primary health care, including chronic disease management, health promotion and disease prevention, to a rostered population of patients in the local community and surrounding area. Two Rivers Family Health Team employs an interprofessional team of health care providers who work in a collaborative model of care.

Two Rivers Family Health Team is currently seeking a 0.8 FTE Social Worker (30 hrs/week) to join its growing team of health care professionals. The position is for 4 days a week. We are an equal opportunity employer and welcome interest from all qualified applicants.

You will work independently and in collaboration with our multidisciplinary team to provide excellent mental health care to a wide variety of patients with a broad spectrum of presenting needs. While carrying a clinical caseload, you will also work with health providers to meet urgent patient needs in the clinic, triage physician and allied health referrals to the FHT mental health program, and assist patients to access appropriate services elsewhere in the community.

You will join our FHT in collaborating with community partners to creatively and effectively meet mental health needs of our patients as well as those of individuals in the wider community.

POSITION DESCRIPTION

We are looking for a Registered Social Worker to join an experienced group of Social Workers. Social Workers on our team work collaboratively with a variety of other health care practitioners including, Family Physicians, Registered Dietitians, Nurses, Pharmacists, Diabetes, Physiotherapists and Occupational Therapists.

QUALIFICATIONS

  • Availability one evening a week is required
  • Excellent clinical skills and experience with assessment, treatment and management of mental health problems commonly seen in primary care
  • Ability to work with a team and the community to create optimal mental health care plans
  • Systems Navigation experience and working knowledge of a wide variety of community resources relevant to mental health and socio-economic challenges
  • Knowledge of Mental Health Act and other relevant legislation
  • Experience with group facilitation. DBT, CBT models of particular interest an asset
  • Experience in crisis/urgent safety planning and interventions
  • Specific experience with Adolescent population an asset
  • Knowledge of electronic medical records (EMR) an asset

Candidates should have a Master’s Degree in Social Work, 2-3 years clinical experience, be a member in good standing with The College of Social Workers and Social Service Workers and possess valid professional liability insurance.
